Camu ‘Mlaen - New Falls Coordinator and Support Workers

Canllaw, in association with the Local Health Board, Hywel Dda NHS Trust and the local authority, has recently appointed a new falls coordinator to work within Canllaw.  Camu ‘Mlaen builds on the success of the falls prevention programme set up and carried out during 2007-9 by Canllaw.  The new officer, Rhian Dawson, said ”This is an exciting opportunity to develop a vital falls prevention service for older people in Carmarthenshire“.  A series of falls events will be held across the county along with the development of a new falls strategy.  Also appointed to work with Rhian are two support workers who will work with her in moving this project forward.

Canllaw Coordinator

 

Following the departure of Huw Dylan Owen from Canllaw in June 2008, the service appointed a new coordinator for an 8 month period, Val McPherson.  The post is now vacant and consideration is currently being given to future service development and management.

Falls Project Wins Award


The Carmarthenshire Falls Project was presented with an award from the Welsh Assembly Government in May 2008 for the work carried out to ensure that the falls events enabled easy bilingual access to services. 

The award adjudicators specifically praised the joint working between the NHS and social services as well as the Local Health Board in Carmarthenshire.

Hywel Dda NHS Trust


The merger of Pembrokeshire & Derwen, Ceredigion and Mid Wales and Carmarthenshire NHS Trusts is called the Hywel Dda NHS Trust.  The new Trust was officially formed on April 1st 2008.

Canllaw Evaluation Report 2007-8


The Evaluation Report on Canllaw from April 2007 to March 2008 is now available as both a short summary document as well as the full technical report.

The report demonstrates financial savings for Carmarthenshire's social services' department as well as for Carmarthenshire NHS Trust.  More important, however, are the case studies within the report which give an insight into the benefits for service users and carers across the county.

An ethnographic study of intermediate care services in Wales: the hidden work

Aim: To explore the perceptions of staff working in and referring to community-based intermediate care teams in Wales. Background

Central and devolved governments have high expectations of intermediate care to promote independence and quality of life for older people and to solve the system pressures within the acute hospital sector. Developing an evidence base of the effectiveness of a model of care that is characterized by diversity and difference in practice is problematic. Method

Fourth generation ethnographic evaluation involving focus groups, non-participant observations and semi-structured interviews with people working within and referring to intermediate care teams. Preliminary findings

Strong evidence of interprofessional working demonstrated by shared responsibility, coordinated rather than parallel services, understanding of other organizations and recognition and resolution of conflict areas. This generated work which traditional measures would not identify. Implications for nursing management

Evaluation is inherent in the work of healthcare professionals and managers. Managers need to aware of the systems and structures within which they are required to manage. This is important for intermediate care services as the heterogeneity contributes to the challenges encountered by managers when seeking to describe services and measure their effectiveness. Applying an appropriate evaluation framework will facilitate both.

Welsh in the Health Services Awards 2007


Canllaw was awarded a prize by the Welsh Assembly Government in 2007 for work carried out in ensuring that the partner agencies provided worked together across traditional boundaries in order to provide accessible bilingual information to the public.
