There are two ways of referring to Canllaw. The first
and easiest is to phone:
0845 600 30 60
and give your referral verbally over the phone.
The second way to refer is to complete the electronic
referral form (click
HERE to download) and e-mail the completed version to canllaw@carmarthen.wales.nhs.uk.
